Nature of in-person coaching

S3 Performance Coaching ("S3") provides supervised 1:1 strength and conditioning sessions delivered in person at the training facility. Sessions involve physical exercise, including resistance training, plyometric and speed work, and sport-specific movement — all supervised directly by your coach.

Assumption of risk

Physical training carries inherent risks, including but not limited to muscle strain, joint injury, and, in rare cases, more serious harm. Participation in sessions is voluntary, and by proceeding you (or your parent/guardian, if a minor) acknowledge and accept these inherent risks.

Facility & supervision

Sessions take place at a training facility that S3 operates from under its own arrangements. Your coach will supervise all exercise directly and adjust technique, load, and exercise selection in real time based on presentation on the day. Standard facility rules and etiquette apply, and clients agree to follow reasonable safety directions from coaching staff.

Emergency procedures

In the event of injury during a session, your coach will provide immediate first aid to the extent trained and available, and will contact emergency services and your emergency contact where appropriate. You confirm the emergency contact details provided are current and accurate.

Medical accuracy & disclosure

You confirm that the health information provided in this form is accurate and complete to the best of your knowledge, and agree to inform your coach promptly of any change in medical status, injury, or medication that could affect training.

Photography & media

S3 may occasionally photograph or film sessions for coaching analysis (e.g. technique review) or, separately, for marketing purposes such as social media or the website. Coaching-analysis use is standard practice; marketing use requires your separate, explicit consent below.

Minors

Where the athlete is under 18, a parent or legal guardian must provide the details and consent below on the athlete's behalf. Junior Athlete Program clients must be accompanied to their first session by a parent or guardian.
